Freshmen

Transfers
- Transfer Supplemental Application Form - Transfer students are required to complete and submit this form. Download the Transfer Supplemental Application Form (PDF file size 44.3kb)
- Request for Mid-Semester Grades Form - In some cases, the Admissions Office may request your mid-semester grades before reviewing your application for admission. If your college produces mid-term grades, simply print them out and staple them to this form. If not, please have your professors list your grades and sign their names on the form. Readmission

Graduate
- Application for Admission for Graduate Study - Applications to graduate programs at Plattsburgh are processed by the Graduate Admissions Office, located on the first floor of the Kehoe administration building. Download Application for Admission to Graduate Study (pdf file size 250kb)
