Transfer Admission Forms

Note: the following forms are available for download. These documents require the latest version of the Adobe Acrobat Reader, which is availabe as a free download.

Transfer Supplemental Application Form

All transfer applicants need to submit the supplemental application.

There is no strict deadline for the supplemental application. However, we encourage you to mail it to the SUNY Plattsburgh Admissions Office within two weeks after you apply. If you have any questions about your application, please call our office at (518) 564-2040 or toll-free at (888) 673-0012.

Request for Mid-Semester Grades Form

In some cases, the Admissions Office may request your mid-semester grades before reviewing your application for admission. If your college produces mid-term grades, simply print them out and staple them to this form. If not, please have your professors list your grades and sign their names on the form.  Upon receipt, your application will be reviewed for admission
