Use this form to authorize Student Accounts to apply financial aid proceeds from one term to pay the balance from another term.

Title IV Authorization Form
Use this form to authorize Student Accounts to apply any credit from Title IV funds in excess of tuition, mandatory fees, room, and meals to pay the remaining balance on your bill.
