Improving Health Services in Our Local Communities
The Eastern Adirondack Health Care Network (EAHCN) is a coalition of agencies and organizations spanning the region of Essex, Clinton and Franklin Counties with a mission to improve the health status of our communities, and the access and delivery of health care in the region, through the collaboration of our members.
Our goals include:
- Increase community involvement and locally generated, innovative solutions to health systems issues in rural areas;
- Improve the financial viability of rural health care providers in the existing health care economy;
- Increase and strengthen cooperation through affiliations, coalitions or partnerships that enhance and coordinate the array of needed health services in rural areas; and
- Strengthen the accountability of local providers for improvements in key public health and/or health care outcomes in the community they serve.
Eastern Adirondack Health Care Network Projects
Healthy Youth Development and Prevention of Substance Abuse and Related Health and Behavior Problems
The Eastern Adirondack Health Care Network's focus on youth and their families began over five years ago with a goal to assist communities and organizations with establishing collaborative partnerships to focus on healthy youth development and prevention of substance abuse, delinquency, teen pregnancy, violence and school dropout. The primary strategy incorporates a public health approach to prevention that aims to influence the entire social environment and integrate prevention activities into the fabric of the community.
