X-rays: done by referral to the Outpatient Department of Champlain Valley Physicians Hospital (CVPH). Students are responsible for all charges incurred at CVPH.

Requirements

The State of New York and SUNY Plattsburgh require that all students enrolled in six or more credits must:

  1. be immunized against measles, mumps, and rubella (MMR);
  2. forward a completed Medical History, Immunization Record, and Physical Examination Report to the Center within thirty days from the first day of classes; and
  3. enroll in the Student Accident and Sickness Insurance Plan or be covered by comparable and adequate insurance through another source. Failure to comply may result in dis-enrollment from the college.

Note that students need a physical from their physician in order to complete the Medical History, Immunization Record , and Physical Examination Report . Also note that if a student is covered by a comparable and adequate insurance company, the student has the opportunity to decline the Student Accident and Sickness Insurance Plan on their bill.

Students who are covered by managed care plans outside of Plattsburgh are recommended to investigate the out-of-network benefits of their plan. If these out-of-network benefits significantly restrict the use of health care services in the Plattsburgh area, then we recommend that the student purchase the Student Accident and Sickness Insurance Plan in addition to their managed care plan.

For the 2008-2009 academic year, the student health fee is $131 per semester for full-time students and $10.92 per credit hour for part-time students. This fee is included on the bill that students receive for each semester.

Students exempt from this fee are:

  1. registered students enrolled in programs that require them to be away from Plattsburgh State campus (such as student teaching, study abroad, residing at the Minor Institute, internships, co-ops, branch campus students and those students taking all independent study courses); or
  2. full-time employees of the college or the Research Foundation at Plattsburgh State who are enrolled in 3 or less credit hours (excludes graduate and dormatory assistantships;
  3. students whose only classes begin after 4:00 p.m.; or
  4. students enrolled in 3 credit hours or less who commute more than 35 miles to campus.

The Center Director or designee will consider extraordinary exemption requests. Refund requests must be made within 30 days of the first day of the semester.
