Health Forms for Students Accepted to SUNY Plattsburgh

New York State Law requires that all new freshman, transfer, and graduate students who are planning to register for at least 6 credit hours are appropriately immunized, receive a physical examination from a doctor, and provide the college with a Health Report and Meningitis Information Response Form.

The deadline for the completion of these forms is as follows:

  • Fall Semester - July 1
  • Spring Semester - January 1

Please note: Students who pay their deposit after these dates are expected to return these forms prior to the beginning of the semester.

If you haven't already done so, please download and fill in the sections that are to be finished by the student. Once you have done so, make an appointment with your doctor to complete the other sections of the Student Health Report.  Once both forms are completed, mail them to:

Student Health Report

Instructions: Please read, print, complete and return to Center.

Please note: Branch Campus students only need to complete the immunization record section and the meningitis section of the Health Report. Main Campus students must complete all sections.

Meningitis Response

Instructions: New York State Public Law states that all students enrolled in 6 or more credit hours complete this form for their medical records. Please read the Meningococcal Disease Sheet and then complete the Meningitis Response form. You can mail, fax or email the printed form to the Center (see addresses below)..

Please note: This form is required for all students, Main Campus and Branch Campus, enrolled for at least six (6) semester hours or the equivalent per semester.
